For initial submission please list all providers, including Licensed Mental Health Professional (LMHPs) and Non-Independently Licensed Clinicians (NILCs), on the LA UHCCP Medicaid Roster Submission Form. For updates to your roster, please use this form for only NILCs which includes provisionally licensed providers as well as certified providers. LMHPs can still be updated via Provider Express utilizing My Practice Info.
